Interstitial & Diffuse Lung Disease Questionnaire for ACCP Members

Dear ACCP member,
The following questionnaire was developed by the Interstitial and Diffuse Lung Disease NetWork at a special conference entitled "Early and Accurate Diagnosis of Interstitial Lung Disease" that was held at ACCP headquarters in 2003. The work that follows is designed to be used by patients before they get in an examination room to facilitate a complete review of systems, occupational history, and medication history. Should you choose to use this in your office practice, the NetWork would welcome your comments about its utility and limitations. We would like this document to be a work in evolution and will post the date of any revisions should they occur.
