After completing my pulmonary and critical care fellowship, I was offered a position of director of MICU in a 600-bed county hospital. For the first several years, I was completely engrossed in improving patient care, teaching of fellows, and institution of guidelines in this unit. I received local recognition for my efforts but realized that I was a "frog in a well."
