Initial Chest Tube Management
Key point: Suction may be used if the lung fails to expand or used immediately.
For clinically stable patient with a large PSP or clinically unstable patient with a large PSP
- Suction not applied immediately.
Step 1: Catheters or tubes may be attached to either a Heimlich valve (good consensus) or water seal device (good consensus) and left in place until the lung expands against the chest wall and airleak resolves.
Step 2: If the lung fails to re-expand, suction should be applied to a water seal device.
- Suction applied immediately.
Step 1: Suction may be applied immediately after chest tube placement for all patients with a water seal system (some consensus).
