Frequent b-agonist use
Inadequate use of inhaled corticosteroids

Prior barotrauma
Hospitalization despite chronic oral corticosteroid use
Psychiatric illness
Recreational drug and alcohol abuse
Diminished ability to sense and respond to airway obstruction
Female sex
Poor socioeconomic status
