If corresponding by fax, e-mail, or mail:
- Provide your full name and, if possible, your ACCP identification number.
- Include your new address, telephone numbers for office and home, fax number, and e-mail address.
- State when the new address will take effect.
- If the address you want published in the ACCP Online Membership Directory is different from your mailing address, include this address in your correspondence and label it "Directory Address."
