Home Care Literature Review: July - December 2006

Hospital at Home:

This survey of subjects participating in the randomized clinical trial of the hospital at home concept found that a higher proportion of patients were statistically more satisfied with treatment in the hospital at home arm than with the acute care hospital. This difference was statistically different in four of nine domains—satisfied with their physician (adjusted odds ratio (AOR) = 3.84, 95% confidence interval (CI) = 1.32-11.19), comfort and convenience of care (AOR = 6.52, 95% CI = 1.97-21.56), admission processes (AOR = 5.90, 95% CI = 2.21-5.76), and the overall care experience (AOR = 2.98, 95% CI = 1.08-8.21—in a validated 40-question survey instrument. Family members of patients treated in hospital at home were also more likely to be satisfied with multiple domains of care. While the patients were actually more satisfied in eight of the nine domains, this reach statistical significance only in the four noted above, predominantly because of sample size. This is another publication from this excellent study supporting the validity of the hospital at home concept.
